Job description
This role leads the Case Management service at Amana Healthcare in Abu Dhabi, overseeing patient-centered care for long-term, post-acute rehabilitation, transitional care, and home healthcare services. The Case Manager provides clinical and operational leadership to ensure continuity of care, patient safety, staff engagement, regulatory compliance, and adherence to western standards of practice. Responsibilities include managing the Case Management team, following up with hospitals and referral sources for patient acquisition, overseeing clinical and operational activities, optimizing patient outcomes and resource utilization, implementing discharge and transition planning, driving quality improvement and risk management initiatives, and fostering interdisciplinary collaboration and engagement across healthcare teams.

Key responsibilities
- Lead and manage the Case Management team to ensure delivery of high-quality, safe, and patient-centered care. This includes assigning responsibilities, mentoring staff, and providing clinical oversight to ensure all team members adhere to best practices and professional standards.
- Develop and maintain strong relationships with hospitals, referral sources, and other stakeholders to facilitate patient acquisition and smooth transitions between care settings. Regularly communicate with external partners to align on patient needs and services.
- Ensure strict compliance with healthcare regulations and professional codes of conduct, including DOH, DHA, and MOH guidelines. Maintain awareness of regional healthcare policies and ensure the team follows all relevant accreditation standards, including JCIA or equivalent.
- Oversee clinical operations to monitor patient safety, optimize care outcomes, and manage patient satisfaction. Implement protocols for tracking patient progress, documenting interventions, and identifying areas for improvement in care delivery.
- Promote effective resource utilization by monitoring staffing levels, skill mix, and workload allocation based on patient complexity and service requirements. Ensure the team is equipped with necessary resources and training to perform effectively.
- Implement and oversee discharge and transition planning processes to ensure continuity of care and appropriate utilization of healthcare services. Collaborate with multidisciplinary teams to coordinate post-discharge follow-ups and home care services.
- Drive quality improvement initiatives by analyzing patient outcomes, conducting risk assessments, and implementing evidence-based interventions. Lead incident reporting, root cause analysis, and corrective action plans to reduce risks and enhance patient safety.
- Foster interdisciplinary collaboration and effective communication between nursing, therapy, medical, and administrative staff. Build trust, engagement, and compassionate relationships with both internal teams and patients to support a culture of excellence in care delivery.
Experience & skills
- Bachelor’s degree in Physiotherapy or Occupational Therapy from a recognized institution. Advanced certifications in case management or healthcare leadership are advantageous.
- Current DOH license or eligibility to practice as a Physiotherapist or Occupational Therapist in Abu Dhabi, ensuring compliance with local regulatory requirements and professional standards.
- Minimum of 5 years of clinical experience in long-term care, rehabilitation, or related healthcare settings, demonstrating hands-on patient management skills and familiarity with complex care cases.
- At least 3 years in a leadership or management role overseeing case management, rehabilitation, or clinical operations teams, with proven ability to mentor, develop, and manage multidisciplinary teams.
- Proven experience navigating healthcare regulations and accreditation standards, including JCIA or equivalent, with a track record of maintaining compliance in operational and clinical practices.
- Strong knowledge of patient-centered care models, discharge planning, continuity of care processes, and utilization of healthcare resources for optimal patient outcomes.
- Demonstrated expertise in risk management, incident reporting, and quality improvement initiatives, with the ability to analyze patient data, identify trends, and implement corrective actions to enhance service delivery.
- Exceptional interpersonal, communication, and collaboration skills, enabling effective engagement with patients, families, interdisciplinary teams, and external stakeholders to support care coordination and organizational objectives.
